Specific Duties and Responsibilities:
Complex Clinical Care:
- Manage and improve health outcomes for patients with multiple complex and chronic conditions.
- Lead and collaborate with a multidisciplinary team, including social workers, case managers, behavioral health specialists, pharmacists, and nutritionists.
- Develop and implement strong transitional care plans, monitoring patients throughout the care continuum, including hospital discharges, ER visits, skilled nursing, and home care.
- Coordinate with inpatient and outpatient facilities to facilitate appropriate discharges and transitions.
- Treat acute exacerbations of chronic conditions within the AbsoluteCare center, such as CHF, dehydration, infections, COPD, and asthma, differentiating when higher care levels are necessary.
- Provide urgent same-day or next-day appointments for discharged patients needing intensified care to prevent readmissions.
- Monitor patients closely to prevent ER visits and hospital readmissions.
- Apply the PCMH model of care, utilizing specialist care only when necessary and maintaining a strong primary care foundation.
- Educate members on disease progression and the importance of advanced directives.
- Practice evidence-based medicine aligned with the PCMH model.
- Licensed Physician in the state of practice.
- Board certified in Internal Medicine or Family Practice.
- BLS certification required.
- DEA registration.
- Dedicated to serving the underserved.
- Work collaboratively and build successful relationships within the team.
- Experience with EMR systems.
- Problem-solving skills and creativity in clinical solutions.
