We are a mission-driven behavioral health organization committed to empowering individuals on their recovery journey. We are seeking an experienced and compassionate Senior Case Manager to provide case management services to individuals with mental health and co-occurring substance use disorders. This role includes managing complex cases, coordinating comprehensive care, mentoring junior staff, and ensuring compliance with Ohio regulatory and Medicaid standards.

Summary of Responsibilities:

• Conduct assessments and develop individualized service plans (ISPs)
• Coordinate care across healthcare, housing, employment, and community resources
• Monitor client progress and adjust plans to support recovery goals
• Manage complex and high-risk cases, including crisis intervention as needed
• Mentor and support Case Managers and peer staff
• Ensure timely, accurate, and compliant documentation in the EHR
• Collaborate with multidisciplinary teams, including clinicians and external providers
• Support discharge planning and continuity of care
• Participate in quality improvement and program development initiatives

Minimum Qualifications:

• Bachelor’s degree in Social Work, Counseling, Psychology, or related field
• 3–5+ years of experience in behavioral health or substance use services
• Knowledge of co-occurring disorders and recovery-oriented care
• Excellent communication, organization, and problem-solving skills
• Ability to manage complex caseloads and prioritize effectively

Preferred Qualifications:

• Ohio licensure (LSW, LISW, LPC, LPCC, or equivalent)
• Previous leadership, supervisory, or mentoring experience
• Experience with Medicaid documentation and billing requirements
• Familiarity with evidence-based practices (e.g., Motivational Interviewing, CBT-informed approaches)
