Position Summary
The After-School Program Case Manager is responsible for coordinating behavioral health and supportive services for children and adolescents enrolled in the Championed Minds After-School Program. The Case Manager provides structured support, behavioral guidance, skill-building activities, and care coordination to assist youth in improving their social, emotional, and academic functioning.
This position works closely with youth, families, schools, and clinical staff to ensure each child receives comprehensive support that aligns with their individualized treatment goals. The Case Manager also assists with program facilitation, documentation, and monitoring progress toward treatment and behavioral outcomes.
Essential Duties and Responsibilities
Case Management & Care Coordination
- Provide case management services to youth participating in the after-school program.
- Assist in identifying behavioral health, educational, and social needs of program participants.
- Coordinate services between families, schools, therapists, and community providers.
- Conduct regular check-ins with youth to monitor emotional, behavioral, and academic progress.
- Provide resource linkage for families including community supports, educational services, and mental health resources.
Program Support & Youth Engagement
- Assist with implementing structured after-school programming including academic support, life skills, and social development activities.
- Facilitate group activities focused on social skills, coping strategies, conflict resolution, and emotional regulation.
- Support youth in completing homework and educational enrichment activities.
- Promote positive peer interactions and reinforce appropriate behavioral expectations.
- Maintain a safe, supportive, and trauma-informed environment for youth.
Behavioral Support
- Assist youth in developing coping strategies and problem-solving skills.
- Provide behavioral redirection and support using trauma-informed and strength-based approaches.
- Monitor youth behavior and intervene appropriately during conflicts or challenging situations.
- Communicate behavioral concerns to clinical staff and caregivers when necessary.
Documentation & Compliance
- Maintain accurate and timely documentation of services in the agency’s electronic health record (EHR) system.
- Document daily program participation, case management contacts, and behavioral observations.
- Ensure documentation meets agency standards and applicable regulatory requirements.
- Assist with maintaining participant records and program attendance logs.
Family Communication
- Maintain regular communication with parents/guardians regarding youth participation and progress.
- Provide support and education to caregivers regarding behavioral strategies and program expectations.
- Participate in family meetings when appropriate.
Collaboration
- Work collaboratively with therapists, program staff, and leadership to support youth treatment goals.
- Participate in staff meetings, supervision, and case consultations.
- Communicate any safety concerns, incidents, or changes in youth functioning to leadership.
Minimum Qualifications
- 3+ Years Experience working in the field
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, Education, or a related field preferred.
- Associate degree with relevant experience working with youth may be considered.
- Experience working with children and adolescents in behavioral health, education, or youth development programs preferred.
- Knowledge of trauma-informed care and behavioral management strategies.
- Ability to work effectively with diverse populations and families.

Work Schedule
Typical hours align with the after-school program schedule, which may include:
- Monday – Friday
- Afternoon and early evening hours
- Occasional evening or special program events as needed.
