Medical Director – Skilled Nursing & Long-Term Care

Corporate Hiring Solutions Inc. is seeking an experienced Medical Director / Physician Medical Director to provide clinical leadership, medical oversight, regulatory guidance, and physician supervision for a 242-bed skilled nursing and rehabilitation facility , including a 32-bed enhanced memory care unit serving Veterans, Veteran Spouses, and Gold Star Parents in Oxford, New York.

This is an excellent opportunity for an experienced physician with a background in skilled nursing, long-term care, geriatrics, nursing home medicine, clinical leadership, and medical administration .

  1. Generally Monday–Friday, 8:00 AM–4:30 PM
  2. On-site at least 1–2 full working days per week
  3. Additional weekend availability may be required as determined by the facility
  4. On-call availability outside scheduled onsite hours as needed for emergencies and regulatory requirements
  5. Must be available to respond to CMS, New York State Department of Health (DOH), VA, or other unannounced regulatory/inspection surveys when required

Position Overview

The Medical Director will provide comprehensive clinical leadership, medical oversight, physician supervision, quality assurance, regulatory compliance, and medical administrative support for residents of a skilled nursing and long-term care facility.

The Medical Director will work closely with facility administration, nursing leadership, attending physicians, Advanced Practice Providers (APPs), interdisciplinary care teams, and other healthcare professionals to promote high-quality resident care and compliance with applicable federal, New York State, CMS, DOH, VA, Medicare, Medicaid, and skilled nursing facility requirements .

Key Responsibilities

Clinical & Medical Leadership

  • Provide medical leadership and oversight for resident care within the skilled nursing and long-term care environment.
  • Assume administrative authority and responsibility for implementation of medical services, policies, and procedures.
  • Coordinate medical care and emergency treatment procedures.
  • Serve as a primary medical liaison with attending physicians and healthcare providers.
  • Monitor the quality, scope, and appropriateness of medical services provided to residents.
  • Promote continuous surveillance of resident health status and quality of care.
  • Ensure medical treatment and regimens are appropriately incorporated into resident care plans.
  • Assist with development and implementation of resident transfer and emergency procedures.
  • Support continuity of medical services and arrange qualified physician coverage during periods of absence when necessary.

Skilled Nursing & Geriatric Care

  • Provide clinical oversight consistent with the needs of a Medicare- and Medicaid-certified skilled nursing facility and licensed long-term care facility.
  • Support high-quality care for geriatric and medically complex residents.
  • Provide guidance regarding long-term care, rehabilitation, memory care, chronic disease management, and resident care planning.
  • Maintain awareness of current geriatric medicine practices and applicable healthcare regulations.

Regulatory Compliance & Quality Assurance

  • Monitor compliance with applicable federal, state, local, CMS, DOH, VA, and accreditation requirements.
  • Participate in Quality Assurance and Assessment Committee activities and other facility committees as assigned.
  • Review inspection and survey reports and provide recommendations to facility administration.
  • Support medical record, incident reporting, accident reporting, and resident care management processes.
  • Identify significant or chronic clinical concerns and assist administration in developing corrective action plans.
  • Monitor compliance with attending physician responsibilities and established facility policies.
  • Assist with physician and healthcare professional credentialing and quality monitoring.
  • Respond to regulatory inspections and surveys, including unannounced CMS/DOH/VA inspections.

Policy Development & Staff Education

  • Participate in the development and annual review of medical, nursing, resident-care, and healthcare policies.
  • Help ensure policies address the comprehensive needs of residents.
  • Develop and participate in in-service education and training programs for nursing and other healthcare personnel.
  • Provide guidance regarding employee health policies and workplace-related medical concerns when appropriate.

Interdisciplinary Collaboration

  • Collaborate with administrators, nursing leadership, attending physicians, APPs, nurses, therapists, social workers, and other interdisciplinary team members.
  • Participate in resident care planning and quality improvement initiatives.
  • Maintain effective communication with facility leadership and healthcare providers.
  • Serve as a medical representative of the Veterans Home within the community when appropriate.

Required Qualifications

  • Active New York State medical license and current registration to practice medicine.
  • Physician with demonstrated experience serving as a Medical Director, physician leader, medical administrator, or clinical supervisor .
  • At least 2 years of experience acting in a medical administrative or supervisory capacity for the Medical Director role.
  • Experience with skilled nursing facilities, long-term care, nursing home medicine, geriatric medicine, rehabilitation, or hospital-based skilled nursing/long-term care is highly relevant.
  • Strong understanding of CMS, New York State Department of Health, Medicare/Medicaid, VA, skilled nursing facility, and long-term care regulatory requirements .
  • Demonstrated ability to collaborate with attending physicians, nursing leadership, administration, and interdisciplinary healthcare teams.
  • Strong clinical judgment, leadership, communication, documentation, and organizational skills.
  • Ability to provide on-call consultation and respond to urgent or emergent clinical and regulatory needs.

Preferred Qualifications

  • Board certification or demonstrated experience in Geriatric Medicine, Internal Medicine, Family Medicine, or Long-Term Care .
  • Previous Nursing Home Medical Director / SNF Medical Director experience.
  • Experience with Veterans healthcare or VA-related services .
  • Experience with CMS and DOH surveys, regulatory compliance, quality assurance, and nursing home inspections .
  • Continuing education in geriatric medicine.
  • AMDA (Society for Post-Acute and Long-Term Care Medicine) certification or comparable long-term-care medical-director training is a plus.

About the Opportunity

The position supports a New York State Veterans Home with a licensed capacity of 242 beds , including enhanced memory care services. The facility primarily serves long-term residents, with a smaller population of short-term/subacute care residents. The facility anticipates maintaining a high occupancy level and serves a significant population of Veterans and their families.

The successful Medical Director will play a key role in maintaining clinical quality, resident safety, regulatory compliance, physician coordination, quality assurance, and continuity of care .
