Shift: open Reports To: Clinical Director Supervises: n/a Position Summary Setting: Residential treatment program operating in a 24/7 milieu. Population: Adolescents ages 11–18 presenting with mood, anxiety, trauma-related disorders, conduct disorder/ODD, and substance use/co-occurring disorders. Unit Census: Unit typically houses 10–16 youth. 16 Max Caseload Expectation: Serve as the primary clinician for approximately 8 youth, providing individual therapy and covering groups. Family Services: Family services as needed per the youth's case plan. Schedule: This position has flexible hours with some evenings and weekends required for programming coverage and unit needs. Position Summary (Clinical) The therapist provides high-engagement, evidence-based psychotherapy and assumes clinical case responsibility for adolescents in residential treatment with co-occurring psychiatric and or non-primary substance use disorders. Key responsibilities include assessment, treatment planning, delivery of individual and group therapy, active engagement in the milieu to support generalization of skills, crisis intervention, interdisciplinary coordination, discharge planning, and timely completion of clinical documentation. Essential Duties & Responsibilities Direct Clinical Care
- Provide individual psychotherapy to an assigned caseload of approximately eight residents, with at least two sessions per resident each week. Frequency may increase for new admissions, crisis follow-up, or higher acuity cases.
- Facilitate and/or co-facilitate structured treatment groups as part of shared coverage, focusing on CBT-informed skills, DBT-informed skills, emotion regulation, distress tolerance, anger management, trauma-informed coping, relapse prevention/recovery skills, and psychoeducation.
- Maintain daily therapeutic presence on the unit, including brief check-ins, skills coaching, and reinforcing treatment goals with residents and staff.
- Complete or assist with biopsychosocial assessments and ongoing clinical reassessment.
- Develop and maintain individualized treatment plans with measurable objectives, targeted behaviors, and discharge criteria, and update plans in accordance with program timelines.
- Contribute to behavior support strategies and safety planning in coordination with direct care and milieu staff.
- Participate in treatment team meetings, clinical staffing, and case consultations.
- Coordinate with psychiatry/medication management, nursing, education, case management, and milieu staff to ensure consistent interventions throughout the day.
- Support discharge and step-down planning by preparing summaries, making referrals, and coordinating continuity of care, as applicable.
- Provide crisis intervention and clinical guidance during escalations, adhering to program policies and training.
- Document and clinically follow up on incidents, risk concerns, and stabilization plans.
- Will participate in onboarding new hire employees by facilitating crisis de-escalation training during new hire orientation and ongoing, as needed.
- Complete documentation accurately and on time, including individual notes, group notes, treatment plans and updates, discharge summaries, and incident/crisis documentation.
- Maintain confidentiality and compliance with HIPAA and, where applicable, 42 CFR Part 2 regulations.
- 48-hour turnaround time for note completion.
- Individual sessions: 8–10 per week, with a baseline of eight sessions (one per assigned resident) plus up to two additional sessions for acuity, admissions, or crisis follow-up.
- Group coverage: Facilitate or co-facilitate 6–8 groups per week.
- Milieu engagement: Daily brief check-ins and skills coaching, documented as required by program standards.
- Treatment team/staffing: Attend 1–2 meetings per week plus brief daily huddles.
- Documentation: Complete documentation on the same day whenever possible; adhere strictly to required deadlines.
- 8:00–8:20 AM: Clinical huddle (review safety, admissions/discharges, high-risk cases)
- 8:20–10:20 AM: Individual therapy block (two sessions)
- 10:30–11:30 AM: Group session (skills/process; co-occurring focus)
- 11:30–12:00 PM: Documentation block
- 12:00–1:00 PM: Lunch
- 1:00–2:00 PM: Individual session or assessment/treatment planning/new admission work
- 2:00–3:00 PM: Group session (CBT/DBT skills, relapse prevention, coping)
- 3:00–4:30 PM: Milieu engagement, care coordination, and documentation wrap-up
- Master’s degree in social work or a qualifying related field, with an active Ohio LSW in good standing.
- Experience working with adolescents in behavioral health settings (internship or practicum experience is acceptable).
- Ability to deliver structured, evidence-based interventions, including CBT-informed approaches, DBT skills, motivational strategies/MI, and trauma-informed care.
- Strong documentation, time management, and interdisciplinary collaboration skills.
- Experience in residential settings with adolescents and co-occurring disorders.
