Assist the Social Service Director in completing social service assessments, participate in care planning, meet with residents and families, assist in interpreting resident social, psychosocial and emotional needs and support, provide information to resident/families as to Medicare/Medicaid coordinate discharge planning and referrals, document resident concerns or grievances, participate in care conferences, coordinate services with outside agencies as needed maintain confidentiality and comply with HIPPA and all applicable federal and Ohio state regulations
