Licensed Practical Nurse (LPN) – Full-Time

?? Schedule: Monday–Friday | 8:30 AM – 5:00 PM

About the Role:
• We’re seeking dedicated Licensed Practical Nurses (LPNs) to provide in-person and telehealth nursing services for clients in Halfway Houses and Intellectual & Developmental Disabilities (IDD) programs across Ohio. This role involves performing health assessments, coordinating care, and promoting client wellness in a supportive, professional environment. Ideal candidates are compassionate, organized, and comfortable working independently.

Key Responsibilities:
• Conduct nursing and health assessments; monitor and record client conditions.
• Document physician orders, medications, and treatments per DODD and ODH standards.
• Administer prescribed medications, including long-acting injectables.
• Update individualized care plans based on client needs and provider input.
• Educate clients and families on health maintenance and disease prevention.
• Schedule and coordinate medical and MAT appointments; accompany clients as needed.
• Collaborate with physicians, specialists, and interdisciplinary teams.
• Support infection control and staff training initiatives.
• Maintain medical supplies, provide first aid, and assist with immunizations.
• Complete reports and referral tracking as assigned.

Qualifications:
• Active LPN license in the State of Ohio
• Associate degree or diploma in Nursing
• 2+ years experience in mental health, IDD, or corrections
