RN Care Coordinator

We're looking for a driven and detail-oriented professional to join our team as an RN Care Coordinator. In this role, you'll play a vital part in providing efficient delivery of coordinated care in an ambulatory, primary care clinic setting to defined patient populations through collaboration with the patients, their family/caregivers, providers (internal and external), insurers, and Third Street staff. The ideal candidate values continuous learning, leads with a welcoming spirit, takes ownership of their work, and is passionate about supporting people and building stronger communities.
Essential Job Duties:
- Provides care coordination and support for patients with chronic or high-risk health conditions (e.g., diabetes, COPD, CHF, prenatal, substance use disorder, homelessness).
- Collaborates with primary care providers and the care team to develop and implement individualized treatment and self-management plans.
- Monitors patient progress toward established health goals and adjusts care plans as needed.
- Connects patients with community resources and coordinates referrals to ensure continuity of care.
- Conducts holistic assessments and patient education to promote engagement in health management.
- Utilizes data and care gap reports to identify at-risk patients and improve overall care outcomes.

Requirements:Qualifications:
- Valid driver’s license and reliable transportation
- Current life support certification or ability and willingness to obtain within 90 days of hire
- Active, unrestricted license as a Registered Nurse (RN)
- 3 Years in a nursing related role
- Knowledge of clinical information systems/software processes, care planning processes, utilization of quality data, regulatory standards, and patient assessment.
- Bachlor's or Master's degree in Nursing preferred.
- Background in Population Health preferred.
- Experience in health care management preferred
- Experience utilizing a electronic health record system
