Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience required. Must have at least 12 months of Home Health experience within the past 3 years (client-specific requirement).
• Experience with HCHB electronic medical record is required (client-specific requirement).
• Ability to meet productivity expectations: minimum 30 productivity points per week (approximately 6 points per day) (client-specific requirement).
• Valid driver’s license, reliable transportation, and ability to travel within assigned territory covering Trumbull, Mahoning, and Columbiana counties (client-specific requirement).
• Preferred: prior experience managing a home health territory and familiarity with home safety assessments.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday–Friday, 8:00 AM–5:00 PM, 8-hour shifts.
• Territory: Trumbull, Mahoning, Columbiana counties.
• Length of Assignment: 13 weeks.
• On-call: No.
• EMR: HCHB required.
• Productivity Expectation: 30 points minimum per week (6 points per day).
