Contract Details
Location: Chillicothe, OH Employment Type: Full-Time Setting: Home Health / Corporate Office Schedule: TBD Shift Options: Days Pay Rate: $40+ (based on experience and assignment) Start Date: ASAP Employment Status: W2 Employee Requirements- Active Physical Therapist (PT) license in OH
- Minimum 1 year of physical therapy experience required (home health experience a plus)
- Minimum 1 year of management experience preferred (home health experience a plus)
- Current CPR certification required
- Ability to work independently in a home setting
- Strong communication and patient care skills
- Valid driver’s license and reliable transportation required
- Ability to pass background screening requirements
- Provide physical therapy services in patient homes per plan of care
- Perform patient evaluations and develop individualized treatment plans
- Implement therapeutic exercises and mobility training
- Educate patients and caregivers on treatment plans and progress
- Monitor patient condition and adjust care as needed
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary care team
