Essential Job Responsibilities:

  • The Community Health Worker (CHW) serves as a trusted resource for pregnant and postpartum women. The CHW helps clients navigate community services, access resources, and adopt healthy behaviors to improve maternal and infant health outcomes in Stark County, reduce disparities, and address social determinants of health.
  • The CHW supports providers and other community partners through an integrated approach to care management and community outreach. Responsibilities include education, social support, advocacy, care coordination, and system navigation through pregnancy and the postpartum period.
  • Travel within Stark County to home visits and community outreach events is required. Identify and recruit new clients. Assist new clients with the enrollment process and appropriate health and social service referrals.
  • Visit clients in their homes or in an agreed upon community setting.
  • Accompany clients to medical/agency appointments as needed and serve as an advocate. Educate clients on the importance and benefits of early prenatal care. Assist with Medicaid enrollment.
  • Provide accurate and relevant health information, including available family planning method choices.
  • Assist clients with the development of personal goals to improve health outcomes.
  • Provide emotional support to clients and/or family during prenatal and early infancy periods. Work with clients to improve parenting skills and recognize needs for change.
  • Promote primary health care for infants/young children and healthy lifestyle changes/choices through education, prevention and modeling behaviors.
  • Demonstrate cultural sensitivity towards client’s needs and beliefs. Assist clients in becoming knowledgeable consumers of health care by empowering them to articulate questions and assert needs and desires.

Minimum Education, Training, and/or Experience Needs:

  • Ability to assume advocacy role for clients and work with at-risk clients in home and/or other settings. Must have good listening skills; be non-judgmental and flexible; and deal with frustration appropriately while maintaining appropriate boundaries. Must demonstrate ability to interact sensitively and professionally with others from diverse cultural, socioeconomic and educational backgrounds.
  • High school diploma, Associate degree in a related health and human field preferred.
  • Valid, Ohio driver's license and auto insurance
  • Same ethnicity and gender as targeted population preferred.
  • Excellent oral communication with individuals within the community is required.
  • Must be dependable and accountable.

Hours: 8:30-4:30 with some weekend and evening availability

Salary: Starting at $33,048, builds based on experience
