Pay Rate: $27.17 per hour with $0.72 mileage reimbursement
Assignment Dates: 06/29/2026 – 12/26/2026
Position Type: Temporary to Permanent (opportunity for extension and/or permanent placement based on business needs)
Location: Field-based throughout Northeast Ohio
Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST, with flexibility to work later hours as needed to meet member needs.
Territory Coverage: Candidates must reside in Ohio and be able to support members throughout Northeast Ohio, including but not limited to Cuyahoga, Summit, Medina, Portage, Marion, Wyandot, Crawford, Sandusky, Stark, Belmont, Jefferson, Huron, Harrison, Lake, Geauga, Lorain, Ashtabula, Ottawa, Erie, Wood, Seneca, Hancock, Union, Trumbull, Mahoning, Columbiana, Carroll, Tuscarawas, and Monroe counties.
Travel Requirements: This is a full-time field-based telework position requiring travel within the assigned region up to 50% or more of the time. Candidates may also be required to travel to the Franklin County corporate office for onsite meetings and trainings, typically 1–2 times per year.
Position Overview:
The Remote Field Case Manager supports children and families with complex behavioral health needs through comprehensive care coordination, case management, member education, and collaboration with multidisciplinary care teams. This role focuses on improving healthcare outcomes, addressing barriers to care, and ensuring members receive appropriate services and support.
Key Responsibilities:
Evaluation of Members:
- Conduct comprehensive evaluations of referred members using care management tools, data review, and available resources.
- Assess member eligibility, healthcare needs, benefit plans, and available internal/external services.
- Identify high-risk factors and service needs that may impact health outcomes.
- Recommend appropriate case resolutions and interventions.
- Coordinate assigned care plan activities and monitor member progress.
Care Coordination & Quality Improvement:
- Collaborate with case managers, supervisors, medical directors, and behavioral health professionals.
- Participate in multidisciplinary case conferences to achieve optimal member outcomes.
- Work closely with Child and Family Teams.
- Identify and escalate quality-of-care concerns through established processes.
- Utilize negotiation and problem-solving skills to secure appropriate services and resources.
- Promote member engagement through motivational interviewing and behavior change strategies.
- Provide coaching, education, and support to empower informed healthcare decisions.
- Serve as a primary point of contact for members and assist with resolving care gaps and access issues.
Monitoring & Documentation:
- Maintain accurate and timely documentation of all case management activities.
- Monitor care plans and evaluate outcomes.
- Ensure compliance with regulatory requirements, accreditation standards, and organizational policies.
Required Qualifications:
Education:
- Bachelor’s degree or non-licensed Master’s level clinician required.
- Degree preferred in behavioral health, human services, health services, public health, psychology, social work, marriage and family therapy, counseling, juvenile justice, or related fields.
Experience:
- Minimum 2 years of experience in behavioral health, social services, or human services.
- Minimum 2 years of experience using personal computers, keyboard navigation, and Microsoft Office applications including Outlook, Word, Excel, and SharePoint.
- Minimum 2 years of experience providing community-based services to children, youth, and families in one or more of the following areas:
- Children's mental health
- Child welfare
- Developmental disabilities
- Juvenile justice
- Public sector human services
- Behavioral healthcare
Specialized Knowledge & Experience:
- Minimum 2 years of experience in one or more of the following:
- Family systems
- Community systems and resources
- Case management
- Child and family counseling/therapy
- Child protection
- Child development
- Minimum 2 years of experience working with Ohio delivery systems, community networks, and local resources.
Additional Requirements:
- Must reside in Ohio.
- Must be willing and able to travel throughout the assigned territory up to 50% of the time.
- Reliable transportation required.
- Mileage reimbursement available according to company expense reimbursement policy.
- Must be willing and able to work beyond standard business hours when necessary.
- Position is patient-facing and requires onsite member visits as needed.
- Verifiable High School Diploma or GED required.
- Safety-sensitive position.

- Case management experience.
- Discharge planning experience.
- Managed care experience.
- Medicaid experience.
