Physical Therapist,

This position is for a Physical Therapist providing skilled home health services to patients in Seaside, OR and surrounding areas. The therapist will manage a caseload of up to six patients daily, conducting assessments, developing treatment plans, and providing direct patient care. The role requires adherence to established productivity standards and utilization of electronic health record systems.

Responsibilities:

- Provide skilled physical therapy services to patients in their homes.
- Develop and implement individualized treatment plans.
- Conduct patient assessments and document findings in electronic health records (EPIC and Oasis).
- Manage a caseload of up to six patients per day, meeting productivity standards.
- Maintain accurate and timely documentation.
- Collaborate with other members of the healthcare team.
- Adhere to all company policies and procedures.
- Manage travel between patient locations within a defined radius.

Required Skills & Certifications:

- Current Physical Therapist license in Oregon.
- Minimum of one year of skilled home health experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis documentation system.
- COVID-19 vaccination (primary series and one booster).
- Valid driver's license, car insurance, and reliable transportation.
- Ability to manage a caseload efficiently and meet productivity targets.

Preferred Skills & Certifications:

- Experience with EPIC electronic health record system.

Special Considerations:

- 8-hour day shifts; no 4x10 or 3x12 schedules accommodated.
- Up to one shift per pay period may be called off by the facility.
- Maximum daily travel radius of approximately 60 miles (60-90 minute drive time).
- Business casual attire; scrubs allowed but not required.
- Facility requires primary series COVID vaccination in addition to one booster.
- First-time travelers are accepted.

Scheduling:

- 8-hour day shifts.
- No weekend requirements.
- No on-call requirements.
- No flexibility for block scheduling.
