Primary Responsibilities

As a field-based LPN, you’ll work closely with patients and care teams to provide high-quality, compassionate care . Responsibilities include:

• Deliver care according to physician instructions and the established plan of care
• Administer medications, wound care, and other approved treatments
• Monitor patient conditions and report significant changes to the RN Case Manager or physician
• Educate patients and caregivers on condition management and self-care techniques
• Document visits accurately using our EMR system
• Assist with specialized procedures as directed by supervising RNs
• Ensure regulatory compliance and follow all safety protocols

Qualifications

• Active LPN license in Ohio (or eligible to obtain)
• Graduate of an accredited Practical Nursing Program
• Minimum 1 year of LPN experience (home health experience preferred, not required)
• Knowledge of Medicare guidelines and CMS standards is a plus
• Comfortable with electronic charting and managing your own schedule
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

• Coverage Area: Cincinnati, OH
• Work Setting: Patients’ homes
• Physical Requirements: Ability to lift, bend, walk, and stand throughout the shift
• On-Call Availability: Optional rotations for evenings, weekends, and holidays
• Communication: Must promptly notify the Director of Nursing of any schedule changes
