RN Home Health Care Admissions Nurse - Flexible Days / Part‑Time / PRN

Home Care by Callos , is a non -skilled Home Care Agency currently recruiting a Registered Nurse professional for the position of Admissions Nurse . THERE IS NO PATIENT CARE INVOLVED IN THIS ROLE. Must be available Mon-Fri for Day shifts. This position is ideal for a dynamic, self-motivated professional who enjoys being on-the-go and has a passion for making meaningful connections.

Responsibilities of the position include:

• Educate healthcare professionals, facilities, physicians, and community organizations on the benefits and services of Home Care by Callos
• Perform comprehensive nursing assessments for admissions, readmissions, and start of care visits.
• Build and maintain strong professional relationships with Clients and Families.
• Ensure compliance with all regulatory requirements and company policies.
• Must be able to work with private pay patients and CareScout clients
• Paid per visit/ plus mileage
• Develop and implement individualized care plans based on patient needs.
• Complete Carescout assessments accurately and timely.
• Remain active and on the go throughout the day educating patients/conducting training, and community partners.

Qualifications / Experience Requirements:

Must have:

• State Licensed RN
• Experience in a Homecare setting preferred.
• Experience working with elderly patients
• Excellent written and verbal communication skills
• Must be tech savvy
• Strong interpersonal, communication, and relationship-building skills.
• Self-starter with excellent organizational and time -management abilities.
• Ability to work independently while being part of a collaborative team
• Valid drivers license, reliable transportation, and current auto insurance
