Position Type: Full-Time Tuesday-Friday 12pm-8pm Saturday 9am-5pm
Position Overview: We are seeking a highly organized and detail-oriented Discharge Planner (CDCA/PRS) to manage aftercare planning and client transitions. The ideal candidate excels in time management, follow-through, and multitasking within a structured, results-driven environment.
Key Responsibilities: Initiate discharge plans; conduct follow-ups; coordinate referrals (housing, outpatient, medical, recovery support); resolve barriers (housing, employment, legal, financial); maintain documentation; ensure releases/referrals are completed; communicate professionally; collaborate with teams.
Required Skills: Time management; organization; multitasking; accountability; professional communication; problem-solving. Requirements: CDCA or PRS required; SUD or case management experience preferred.
