We are seeking a Licensed Practical Nurse to join our team. In this role, you will be caring for Hospice patients in their homes. You are a Licensed Practical Nurse (LPN) with great attention to detail and a big heart. You take pride in caring for patients following personalized care plans for patients. You are able to assess, observe and communicate effectively regarding your patients care. Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
- Currently licensed Licensed Practical Nurse (LPN) in state of Ohio
- Previous experience as a Licensed Practical Nurse is preferred
- Hospice experience preferred but not required
- Commitment to excellent care and customer service
- Excellent interpersonal and client care skills
- Valid drivers license
- First aid/CPR certification required
