Responsibilities:• Assist clients with daily living activities, including personal hygiene, meal preparation, and medication management.
• Develop and implement individualized care plans tailored to each client's needs and goals.
• Provide emotional support and companionship to enhance clients' quality of life.
• Document client progress and maintain accurate records in compliance with state regulations.
• Collaborate with healthcare professionals and family members to ensure comprehensive care.
• Engage clients in recreational and social activities to promote community involvement.
• Monitor and report any changes in clients' health or behavior to the appropriate personnel.
• Participate in ongoing training and professional development to enhance skills and knowledge.
* May be willing to transport clients to doctor appointments, errands or outings.
