POPULATION HEALTH - PIVOTS PROJECT COORDINATOR
PIVOTS (Providing Investments for Opioid Treatment and Recovery in Sussex) is a new grant-funded project that will expand services for opioid use disorder patients in Sussex County . The Project Coordinator is a key position that will provide overall planning, management, and coordination of the PIVOTS Project .
Additionally, t he Coordinator will be a Advanced Practice Provider responsible for direct patient care in the context of promoting evidence-based treatment protocols . She/he will function as a compassionate change agent for creating a more responsive, effective system of care for opioid use disorder patients .
Responsibilities Key responsibilities include: Oversee the development and implementation of enhanced and/or new services for treatment of Opioid Use Disorder in the emergency department, inpatient units and outpatient settings. Work with clinical and operational team members across the organization to coordinate services and ensure a low-barrier, trauma-informed approach to care. Provide overall project management, develop and maintain project workplan and manage project deliverables and timeline. In partnership with PIVOTS interdisciplinary team and Beebe clinical and operational leaders, develop or modify policies, protocols and workflows necessary to support the delivery of evidence-based services for OUD across multiple systems of care. Collaborate with Information Services to plan and facilitate necessary modifications to electronic health record systems. Work to ensure that systems are in place to link individuals to appropriate prevention, treatment, and recovery support services. Manage all training and technical assistance required by the project. Identify and engage consultants to provide expertise in process improvement for increased MOUD services. Develop scope of work, generate training plan, and oversee external service provider(s) engaged to deliver training. Facilitate project team meetings, assign tasks, monitor progress and coordinate efforts across departments. Develop and maintain systems to track project progress, data, and outcomes. Use data-driven decision-making to guide program modifications and demonstrate impact to funders. Monitor patient and other stakeholder satisfaction. Address patient complaints through established organizational mechanisms. Serve as the primary technical point of contact with the funding agency and other key partners. Act as a visible champion for the project, foster collaboration among Beebe departments, community partners, and other stakeholders to ensure the project's success. In concert with Beebe's Marketing Office, develop and implement an outreach campaign to communicate availability of services and promote appropriate access. Provide limited direct patient care to support the implementation of enhanced OUD services in new settings. Maintain current knowledge and compliance with all relevant regulatory requirements. Maintain privileges, mandatory education, and regulatory requirements as needed for maintenance of licensure. Qualifications Minimum Qualifications Master's degree in nursing from an accredited college or university. Advanced Practice Nurse (APN) / Nurse Practitioner (NP) with acute care license or dual licensed as a FNP and ACNP. Completion of a Nurse Practitioner program specific to the expanded specialty of Psychiatric/Mental Health Nurse Practitioner (PMHNP). Must be licensed in the state of Delaware. At least one year of experience in project management, administration, public health programming, and/or execution of behavioral health programs. Demonstrated knowledge of evidence-based strategies for overdose prevention and harm reduction programs. Must be able to obtain a DEA license. Demonstrated aptitude to exercise compassion for individuals with mental health/substance use disorder and ability to work with patients during their recovery journey and in partnership with the patient, identify treatments that allow for quality of life in a safe and fulfilling way. Act as a role model for staff who help individuals struggling with SUD as well as a change agent for behavioral and/or mental health-related stigma. Experience working in different practice settings including inpatient, outpatient, and mobile health locations. Demonstrated ability to assume a leadership role in managing projects and associated risks related to projects. Proven track record of stakeholder engagement to include working effectively with numerous stakeholders including senior management, clinicians, and community-based partners. Basic computer skills/proficiency with electronic medical records (e.g., EPIC, Athena, and/or Cerner).
