Schedule:
Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary:
At Buckeye PACE, our Physical Therapist (PT) provides services to promote, maintain, and restore Participants' functional mobility and quality of life. They diagnose and treat movement-related disorders and physical impairment by performing comprehensive evaluations utilizing objective measures and prescribing therapeutic exercise, therapeutic activities, gait training, and other physical modalities. PT also provides pain management and medical equipment recommendations and implements/oversees individual and group exercise programs. The PT works as part of an interdisciplinary team (IDT) to create an individualized comprehensive plan of care to meet participant goals and needs.
Duties/Responsibilities:
Conducts initial and ongoing assessments and evaluations as required by Policy and Procedure and as recommended by IDT.
Perform a review of medical history and complete a comprehensive physical therapy assessment to develop a plan of care, establish goals, and provide treatment programs.
Recommend appropriate durable medical equipment, orthotics, and prosthetics to maximize the functional mobility of participants.
Records and signs all treatments in Participant medical records.
Provides training to family members, caregivers, and staff members.
Plans and develops programs for restorative and wellness programs.
Supervise physical therapist assistants.
Actively functions and participates as a member of the interdisciplinary team.
Provides visits and/or treatment in various settings, including day centers, Participants’ homes, skilled nursing facilities, or hospitals.
Communicates Participant changes to team members as appropriate.
Participates in the coordination of 24-hour care delivery.
Must develop and maintain a strong working relationship with facility administration and staff, contract providers, other healthcare professionals, participants, caregivers, and families.
Participate in Quality Initiative training as needed.
Other duties as assigned.
Required Skills/Abilities:
Must be able to deal tactfully and patiently with other staff, participants, family/caregivers, visitors, government agencies/personnel, and the public.
Must be able to adapt well to change.
Need to have exceptional time management and problem-solving skills.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must have the cognitive ability to process and perform tasks for Participant safety.
Reliable transportation and a clean driving record.
Ability to prioritize work and follow through on assigned tasks.
CPR and First Aid Certification.
Excellent written and verbal communication skills.
Education and Experience:
Must hold a current PT license within the state you are practicing in.
One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
Demonstrate clinical competency with the geriatric population.
Requirements:
Successful completion of background check, physical, drug screening, TB Screening.
Physical Requirements:
Sits, stands, bends, lifts, and moves intermittently during working hours.
Is subject to lifting, carrying, and supporting participants and equipment.
Must be able to drive a motor vehicle.
Is subject to frequent interruptions.
