Job Description

Expert Practice

Provides expert nursing to post heart transplant patients by maintaining:

  • Knowledge and care of post-heart transplant patients.
  • Ability to interpret data about the patients health and transplant status in order to provide appropriate nursing care.
  • Knowledge of medications and their correct administration based on age of the patient and their clinical condition including the five rights.
  • Patient care provided based on acuity/urgency of patient needs preference choices and available resources.
  • Knowledge of the principles of growth and development over the life span and skills necessary to provide age appropriate care to the patient population.
  • Ability to interpret data about the patients status post-transplant in order to identify each patients age specific needs and provide care needed by the patient group.

Nursing Process

Systematically assesses the need for and the delivery of nursing services that meet patient care requirements whether delivered directly or by the other nursing staff through:

  • Ability to perform a systematic assessment on post heart transplant population.
  • Ability to adequately assess and reassess pain using appropriate pain management techniques for post-transplant patients and educate the patient and family regarding pain management
  • Meeting documentation standards and policies including UNOS transplant database.

Patient CAre Coordination

Ability to formulate coordinate and revise an individualized plan of care as indicated by the patients response to treatment and evaluate overall plan daily for effectiveness by:

  • Treating patients and their families with respect and dignity. Identifying and addressing psychological cultural ethnic and religious/spiritual needs of patients and their families.
  • Functioning as liaison between administration patients physicians and other healthcare providers.

Telephone Communications

Provides care for individual patients over the telephone using the nursing process by:

  • Consistently following policies and procedures for providing telephone medical advice to post transplant patients.
  • Consistently documenting care provided via telephone per UWMC policy.
  • Providing efficient and effective telephone triage for post heart transplant patient population.

Patient Safety

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors by:

  • Complying with specific UWMC policies for safety and infection control.
  • Utilizing PSN to report events causing or having the potential to cause patient harm.

Client TEaching

Integrates patient and family education into the delivery of care ensuring educational activities are culturally appropriate and are tailored to the patients and familys ability to comprehend and participate in the post-heart care by:

  • Formulating a teaching plan based upon identified learning needs and evaluating effectiveness of learning family is included in teaching as appropriate.

Enabling Operations

Coordinates the care given by others to achieve efficient clinic operations by:

  • Communicating appropriately and clearly to manager co-workers physicians and all members transplant team.
  • Demonstrating an ability to be flexible organized and function well under stressful situations.
  • Orienting new permanent and temporary personnel to the transplant team. Serving as a preceptor and mentor.
  • Adhering to Attendance Standards.
  • Demonstrating knowledge of department specific billing policies and application of their procedures including Facility Only Coding and nurse visits.
  • Demonstrating ability to use computer applications required for successful completion of job requirements including EPIC Transplant database UNET and other UWMC programs.

Professional Growth

Assumes responsibility for continued professional growth and sharing expertise with other team members by:

  • Teaching other health care team member(s) using informal and formal methods.
  • Sharing expertise and information formally and informally such as publication presentation or community activities.
  • Participating in professional organizations and/or is responsible to develop or maintain own professional growth.
  • Participating in staff meetings in person and/or actively seeking to keep self-informed of information.

Qualifications

  • Bachelors of Science Degree in Nursing
  • Active unrestricted Washington State Registered Nurse License
  • Minimum of 1-2 years of cardiac-related nursing experience in a cardiac ICU and/or Cardiac Telemetry Unit
  • 1 year experience as a post-heart transplant nurse coordinator preferred
  • ACLS certification preferred
  • BLS certification required

Compensation Benefits and Position Details

Pay Range Minimum:

$45.59 hourly

Pay Range Maximum:

$83.64 hourly
