As our RN Case Manager , you'll orchestrate meaningful care that touches hearts and creates lasting comfort for families during their most vulnerable moments. This role goes beyond medical tasks – you'll be the steady, compassionate presence that makes all the difference.

Your Impact Zone: As an RN Case Manager , you'll travel throughout the Dayton area, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable.

What You Bring & What We Offer

Required Qualifications:

  • Active RN License
  • Valid Driver's License
  • 1+ years hospice experience

Preferred Qualifications:

  • BLS Certification
  • Hospice and Palliative Medicine background
