Primary Care Nurse Practitioner Cleveland, OH
COMPENSATION: Salary $140k + $10k Bonus
Our client is a physician-led, mission-driven primary care organization dedicated to transforming healthcare for seniors through high-quality, value-based care. Role:
· Compensation: $140k
· Bonus: Up to 10k year 1 and goes up as a partner.
· 18-20 patients a day
· Hours: Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15)
· Work/Life Balance
· Small Patient Panel 450 patients max
· Administrative time included in schedule
· No nights or weekends
· 100% outpatient
· Dedicated Care and support team (have your own MA, Scheduler & Referal Speecialist
· Patient Population: Seniors with 3-5 chronic diseases

Clinic offers:
· Inhouse Consulting Specialist: Cardiology, Podiatry and more

Requirements:
· Board Certified Nurse Practitioner
· 2 + years NP outpatient primary care experience
· Geriatric experience (preferred)
