Clinical Director Position Summary:
The Case Manager is a vital member of the multidisciplinary team, responsible for delivering high-quality, client-centered case management services within our Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), and Outpatient Program (OP). This role focuses on providing holistic, community-based care to individuals with co-occurring mental health and substance use disorders, ensuring that each client receives comprehensive support that promotes recovery, wellness, and long-term stability. Key Responsibilities:
- Client-Centered Support:
- Conduct comprehensive needs assessments and develop individualized service plans in collaboration with clients, families, and the treatment team.
- Advocates for client needs and preferences, ensuring their voice is central in all decision-making processes.
- Holistic Care Coordination:
- Link clients to appropriate community resources, including housing, vocational support, healthcare, peer support groups, and social services.
- Coordinate care with mental health, medical, and substance use providers to ensure seamless integration of services.
- Treatment Planning & Documentation:
- Participate in treatment team meetings to review progress, update goals, and modify plans as needed.
- Maintain accurate, timely, and confidential documentation in accordance with program policies and regulatory standards.
- Community Integration:
- Foster relationships with local organizations, agencies, and support networks to expand available resources for clients.
- Promote community engagement opportunities that support recovery, personal growth, and social connectedness.
- Crisis Support & Intervention:
- Provide crisis intervention, de-escalation, and support during periods of acute need.
- Collaborate with the clinical team to ensure appropriate follow-up care and safety planning.
- Knowledge of co-occurring mental health and substance use disorders, recovery-oriented systems of care, and trauma-informed practices.
- Strong communication, organizational, and problem-solving skills.
- Ability to work collaboratively in a multidisciplinary team and independently in the community.
- Familiarity with local resources, benefits systems, and community supports.
- Valid driver’s license, reliable transportation, and willingness to travel within the community.
- Commitment to client-first care, treating every individual with dignity, respect, and compassion.
- Ability to view treatment through a holistic lens , addressing physical, emotional, social, and spiritual needs.
- Strong engagement and rapport-building skills with diverse populations.
- Flexible, adaptable, and able to respond effectively in a fast-paced, changing environment.
