Caring for Kids Clinical Case Managers (CCM) provide case management and behavioral health services to children in foster care, foster to adopt, and public adoption placements. In addition, they provide support to foster and adoptive families. CCMs have an office on site, the ability to work from home after a training period, and field appointments. This job requires licensure in the State of Ohio (LSW, LISW, LPC, LPCC or equivalent) PRIMARY RESPONSIBLITIES
Completion of Diagnostic Assessments, Individualized Service/Treatment Plans and corresponding documentation as applicable and appropriate.
Work in conjunction with a multidisciplinary team.
Advocate on behalf of clients, including assessing needs and linking to services using contacts with community agencies, monitoring access to those services, and maintaining confidentiality.
Coordinate services for clients in accordance with service description, contract requirements of the custodial county, and accreditation standards.
Face-to-face contact with clients, including required home visits with clients to assess progress and assure child safety.
Assist foster families in the adjustment of a new foster or pre-adoptive child placement in their home and with ongoing support services throughout placement.
Provide TBS/CPST to foster children identified as a specialized or treatment level on the referral and ICCA as well as those in pre- adoptive placements.
Provide consultation and collaboration with foster and adoptive parents on identifying problem areas, developing plans for addressing those problems in order to minimize the risk of placement disruption.
Utilize and educate connected parenting approaches with families. Provide ongoing training in order to continue to develop the skill level of foster and adoptive parents.
Provide on call support and crisis intervention to foster families on a rotation with other team members.
Participation in team decision-making meetings regarding the childs development, progress, and permanency planning.
Complete reports and documentation within the time frames specified according to foster care contract requirements and the agencys accreditation standards.
Report to supervisor and county any incidents that jeopardize the health and safety of children.
Weekly group supervision and individual supervision, with additional support as needed.
Follow guidelines of social work or counselor code of ethics.
Follow all policies and procedures established by Caring for Kids, Inc.
Perform other duties that support the mission of the agency. QUALIFICATIONS
Minimum of Bachelor's in Social Work or Masters degree in counseling
Licensed by the Ohio Counselor, Social Worker and Marriage and Family Therapist Board
Valid Drivers license and car insurance as well as a reliable vehicle with valid registration and a clean driving record KNOWLEDGE AND SKILLS REQUIRED

Demonstrate written and verbal communication skills
