Position Summary

The Wellness Director is responsible for planning, organizing, directing, and evaluating the overall health and wellness programs for residents in a long-term care facility. This role ensures the delivery of high-quality resident-centered care in compliance with federal, state, and local regulations, while promoting residents’ physical, emotional, and social well-being.

Key Responsibilities

  • Clinical Leadership & Oversight
  • Oversee daily clinical operations, including nursing and wellness services.
  • Ensure individualized care plans are developed, implemented, and updated.
  • Monitor residents’ health status and coordinate with physicians, therapists, and other healthcare providers.
  • Conduct regular health assessments and wellness checks.
  • Ensure accurate and timely clinical documentation.
  • Regulatory Compliance
  • Maintain compliance with all federal, state, and local regulations (e.g., Centers for Medicare & Medicaid Services (CMS), state Department of Health).
  • Prepare for and participate in surveys and inspections.
  • Develop and maintain policies and procedures consistent with regulatory requirements.
  • Oversee infection control and safety protocols.
  • Staff Management
  • Recruit, hire, train, supervise, and evaluate nursing and wellness staff.
  • Ensure appropriate staffing levels to meet resident needs.
  • Provide ongoing education and professional development opportunities.
  • Foster a positive, collaborative work environment.
  • Resident & Family Engagement
  • Communicate regularly with residents and families regarding care plans and health concerns.
  • Address and resolve resident or family concerns promptly.
  • Promote wellness initiatives that enhance quality of life.
  • Quality Assurance & Performance Improvement
  • Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Monitor clinical outcomes and implement corrective action plans as needed.
  • Track key performance indicators such as falls, medication errors, infections, and hospital readmissions.
  • Budget & Resource Management
  • Assist in developing and managing the department budget.
  • Monitor medical supplies and equipment inventory.
  • Ensure cost-effective use of resources without compromising care quality.

Qualifications

Education

  • Graduate of an accredited school of nursing (BSN preferred).
  • Current Registered Nurse (RN) license in good standing (state-specific).

Experience

  • Minimum 3–5 years of clinical leadership experience in long-term care, skilled nursing, or assisted living.
  • Experience with electronic health records (EHR).
  • Strong knowledge of geriatric care practices and regulatory standards.
