The Intake SSA answers SSA intake calls and responds and appropriate. The intake SSA position is the gateway to receiving services from the Stark County Board of DD. This position handles all initial calls requesting service and maintains a spreadsheet to include the concerns and outcome(s), and conducts assessments to determine eligibility.
- Answers SSA Intake calls, respond as appropriate, and documents in tracking and case notes
- Assists in gathering necessary documentation for individuals to receive services
- Conducts assessments to determine eligibility, including researching and reviewing diagnosis to verify it meets the definition of developmental disability according to ORC.
- Facilitates the Waiver Enrollment process for HCBS Waivers
- Administers Level of Care assessment and enters into State system
- Works collaboratively with other SSAs and outside agencies to obtain necessary documentation
- Links people to services within the Agency and outside of the Agency.
- Receives and reports on assignments requiring follow-up and outcomes of conducted eligibility assessments.
- Assists with Medicaid applications, enrollment, and follows up as necessary
- Assists in the administration of the Family Support Services (FSS) Program
- Completes Pre-Admission Counseling and follow up for individuals considering residing in Intermediate Care Facilities
- Assists in the administration of the waiting list for Home and Community Based Service (HCBS) Waivers: Completes State assessment for current need and assists in the identification of alternate resources
- Provides access to and/or knowledge of resources available in the community.
- Completes daily TCM notes to document all services delivered. TCM notes are subject to subpoena. Billable TCM notes are billed to the individual's Medicaid
- Follows all State and Federal rules as well as Board policies.
- Provides crisis assistance to ensure that needed services occur in a critical or unexpected situation (e.g.: arranging for respite, addressing behavioral or medical emergencies, on-call responsibilities, etc.).
- Completes accurate documentation in keeping with requirements of the County Board as well as any applicable Medicaid requirements.
- Reports major unusual incidents to include alleged abuse/neglect in accordance with HB 403; Ohio Revised Code 5123; Administrative Rule 5123:2-17-02; and County Board policies and procedures.

Valid Ohio Driver's License, Bachelor's Degree in related field and experience with individuals who have developmental disabilities.
