Eligibility Referral Specialist II - Long-Term Care
Logan County Department of Job & Family Services is seeking a self-driven, detail-oriented and experienced Long-Term Care Case Manager with experience working in Ohio Benefits to join our Income Maintenance Unit.
The primary duties of this position are Case Management/Customer Service for the Long-Term Care caseload. The person in this position will determine eligibility for customers applying for public assistance, redetermine eligibility and/or assess ongoing needs for the consumer. The employee will also determine initial and/or ongoing eligibility for emergency assistance.
The successful candidate will have a minimum one year of experience working at Job & Family Services as an ERS 2. Must have excellent attendance and a positive attitude. Degree is preferred but experience in similar position will also be considered. Ability to use multiple computer platforms and phone system simultaneous is also required. Case Managers also attend meetings, trainings, conferences, workshops, and webinars as needed, and are responsible for data entry on their cases.
Salary will be commensurate with experience and qualifications. Salary Range: $36,961.60 - $47,881.60. Future pay increases are based on years of service in similar county employment of 4% to 6% annually again based on years of service. We also offer an annual Retention Payment based on years of service from $300.00 - $1,300.00. Logan County offers an excellent benefits package, including health insurance benefits, employee paid dental and vision, paid vacation (after one year of service) or after accumulated if prior countable service, deferred compensation plans, and flexible work hours. Logan County participates in the Ohio Public Employee’s Retirement System.
