Spiritual Care Coordinator

The Spiritual Care Coordinator is responsible for the provision of spiritual care and counseling services to patients and families/caregivers either directly or through coordination of care with other spiritual care coordinators. The Spiritual Care Coordinator will be comfortable in an interfaith setting and have the ability to accept different lifestyles, cultures, beliefs, and values, and have the ability to network with community clergy and congregations and have knowledge of and commitment to the hospice philosophy of care in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and Tasks
Care Management - 60%
Completes timely spiritual assessment and initiation of appropriate services for patients and families/caregivers in the hospice program.
Provides direct spiritual care to patients and families/caregivers. May receive requests for spiritual care consultation anytime of the day or night via telephone or in-person visit.
Attends patient care conference and interdisciplinary group meetings.
Facilitates the development of an interdisciplinary group plan of care to meet identified spiritual needs and provides consultation, education, and support to the interdisciplinary group on spiritual care.
Provides funeral or memorial services for patients as requested and plans periodic memorial services to meet the needs of staff, volunteers, community clergy, and spiritual care coordinators working with the hospice team.
Administration and Reporting - 30%
Performs administrative duties such as completing reports, evaluations, charting, and documentation using electronic medical record, and ensures that all documentation is informative and descriptive of services and care provided and of the patient's response to the care.
Updates spiritual care plans as needed.
Serves as liaison and support to spiritual care coordinator and life plan community chaplains, and documents direct services and ongoing communication with them.
Customer and Community Relations - 10%
Recruits spiritual care coordinators, life plan community chaplains, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
Assists in supervision of spiritual care volunteers when assigned to patients and families/caregivers.
Listens and responds to the needs of patients, families/caregivers, and staff members delivering exceptional customer service.
All other duties as assigned.
Qualifications
Education
High school diploma or equivalent required.
Graduate from an accredited seminary or school of theology; or ordained minister in good status with the denomination from which he/she was ordained without restriction or subject to any disciplinary or corrective action preferred.
Experience
Two years experience counseling or working with individuals, families and caregivers related to death and dying required.
Experience in a home health, hospice, or long-term care healthcare setting preferred.
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet preferred.
Comfortable in an interfaith setting and have the ability to accept different lifestyles, cultures, beliefs, and values.
Other Requirements
Must be able to read, write, speak, and understand the English language.
Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
Working Conditions and Special Requirements
Sitting - Up to 8 hours/day

Lifting, transferring, pushing or pulling patients or equipment/supplies - Up to 25 pounds
Driving - Up to 6 hours/day
Work weekends, evenings, and holidays - As needed for coverage
On-call availability - 24/7 for emergencies
Subject to patients with various disease processes - Occasional
May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses - Work day
