Responsibilities:POSITION SUMMARY: The Housing Specialist facilitates transition into Independent Housing by providing interventions to individuals in all housing settings to maintain tenancy, support housing transitions and prevent homelessness. Provides clinical services to support individuals to obtain and/or maintain a housing setting that best meets the Individuals needs and preferences and supports treatment goals.
ESSENTIAL FUNCTIONS:
- Integrate the PRISMModel® for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention
- Establish and maintain effective therapeutic relationships and creatively engage consumers into discussions and planning around recovery and achievement of personalized goals
- Utilize motivational interviewing approach to support engagement in treatment and housing stability
- Assists individuals with skill development in preparation of transitioning to independent living.
- *Assist individuals in locating and obtaining safe, affordable and culturally appropriate housing.
- Network with and develop professional relationships with community landlords and local housing authorities.
- Demonstrate knowledge of the Department of Housing and Urban Development (HUD). Maintain a relationship with the community housing authorities in relation to HUD vouchers.
- Assist persons served in obtaining housing vouchers, or subsidized housing, including completion of paperwork and accompanying each individual to scheduled housing appointments.
- Support each individual with preparing for and planning a move into new housing i.e. utilities, household item, budgeting, landlord relations, and safety.
- Assist each individual with establishing community linkages that may offer assistance with setting-up and maintaining an individual’s housing.
- Provides ongoing support to maintain independent living, including but not limited to maintaining a safe and clean living environment and promoting conflict management skill building necessary to mediate exchanges with roommates, landlord or other tenants.
- Provide training to program staff related to housing resources, including what documentation is needed to obtain housing.
- Maintain clinical documentation that is clear and concise, reflecting consumer preferences, needs and recovery.
- Provide appropriate documentation regarding face-to-face and telephone contact with consumer, staff, and family members through service delivery notes.
- Adhere to documentation guidelines of per DMH policies and procedures, FHR policies and procedures, and CARF standards.
- Ensure a multidisciplinary approach is taken, using evidence-based practices.
- Practice a trauma informed approach to treatment planning and service delivery.
- Follows MAP policy related to storage, supply, and administration.
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
- A Bachelor's degree in Social Work, Psychology, Rehabilitative Counseling, or a closely related field.
- A minimum of two years of experience working with individuals with SPMI or other mental health conditions, and experience within a housing support setting is a plus.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with clients from diverse backgrounds.
- Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
- A commitment to evidence-based practices and a trauma-informed approach.
- Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record.
