8 Week Contract : Home Health Registered Nurse

$2050/wk plus .50 a mile for patient related travel

Kinnser WellSky Charting experience is preferred

Must have a minimum of 1 year of Home Health experience

Schedule: Monday-Friday 9 AM -5:30 PM

No On-call Or weekend requirements

Service Area: Will cover Brown and Adams Counties

Duties

• Conduct comprehensive patient assessments, including vital signs, physiology evaluation, and medical history review to determine individual care needs.
• Develop and implement personalized care plans aligned with best practices in hospital medicine, post-acute care, and outpatient settings.
• Manage complex cases involving infectious disease care, behavioral health issues, geriatrics, pediatrics, and trauma cases such as Level I and Level II trauma center scenarios.
• Perform diagnostic evaluations and coordinate discharge planning to ensure seamless transitions across inpatient, outpatient, and home settings.
• Monitor patient progress through regular evaluations, utilizing medical terminology and HIPAA guidelines to maintain confidentiality and compliance.
• Collaborate with multidisciplinary teams including physicians, case managers, and managed care providers to optimize patient outcomes.
• Document all patient interactions accurately in medical records and ensure compliance with hospital experience standards and utilization management protocols.

Qualifications

• Valid RN license with current registration in the state of practice.
• Proven critical care experience with a strong background in ICU or emergency room settings.
• Experience working in Level I or Level II trauma centers is highly desirable.
• Demonstrated knowledge of hospital medicine, inpatient and outpatient care processes, including post-acute care and discharge planning.
• Familiarity with medical-surgical nursing, behavioral health, geriatrics, pediatrics, and infectious disease management.
• Strong understanding of medical terminology, physiology knowledge, vital signs monitoring, and diagnostic evaluation techniques.
• Ability to perform comprehensive patient assessments and manage complex case loads effectively.
