Hospice Community Liaison
Crossbridge Hospice is seeking a compassionate and dedicated Hospice Community Liaison to join our team. This role is pivotal in enhancing our mission to provide exceptional end-of-life care and support to patients with life-limiting illnesses and their families. The ideal candidate will be responsible for building relationships with healthcare providers, community organizations, and patients to ensure a seamless transition and understanding of the hospice services we offer.
