Position Overview
Spirit Homecare & Hospice is seeking a licensed Physical Therapist (PT) to provide therapy services in patients’ homes. This role involves assessing and treating patients to restore mobility, improve independence, and enhance quality of life , while working in a flexible schedule and collaborative environment.

Compensation & Schedule

• Start of Care / Resumption Visits: $100 per completed visit (includes travel reimbursement)
• Routine Visits: $55 per completed visit (includes travel reimbursement)
• Flexible Scheduling: Full caseload or PRN assignments based on availability
• Team Environment: Work alongside RNs, PTAs, and interdisciplinary clinicians

Key Responsibilities

• Conduct physical therapy evaluations, including OASIS assessments
• Develop and implement individualized treatment plans with the care team
• Deliver therapy to improve mobility, balance, and safety
• Document care and progress accurately in compliance with Medicare and agency standards
• Coordinate with RNs, PTAs, physicians, and other healthcare professionals
• Provide supervision and clinical guidance to PTAs as required
• Educate patients and caregivers on exercises, mobility, and home safety

Qualifications

• Active Ohio Physical Therapist (PT) license
• Home health experience preferred; SNF or outpatient experience considered
• Familiarity with OASIS and Medicare documentation preferred
• Strong clinical, communication, and time-management skills
• Valid driver’s license, reliable transportation, and proof of auto insurance

Schedule & Service Area

• Flexible hours, Monday–Friday, based on caseload
• Travel required within West Chester Township and surrounding communities
