Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• At least 12 months of home health experience within the past 3 years (client requirement).
• Experience with HCHB (Homecare Homebase) and OASIS documentation required (client requirement).
• Ability to meet client productivity expectations: 30 points per week and availability to provide up to 40 hours per week as required (client requirement).
• Must be able to work Monday through Friday daytime schedule as outlined in assignment details.
• Current professional liability insurance as required by employer and facility policies.
• Preferred: previous experience working with hospice or palliative home health patients.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days - 8 hours, Monday through Friday.
• Typical Work Hours: 8:00 AM – 5:00 PM (client schedule indicates primary daytime hours).
• On-call: Not specified by client.
• Documentation Systems Required: HCHB and OASIS.
• Productivity Requirement: 30 points per week; availability for up to 40 hours per week guaranteed by client.
• Floating: May be required to serve the client’s geographic service area in Marion region; candidate must be willing to travel within assigned service area.
• Positions Available: 1.
• Assignment Length: 13 weeks.
