Case Management Coordinator Highlights

Hours: Monday – Friday, 8am-5pm, with flexibility needed to work later as required.
Pay: $27.16 per hour

• Some travel required within Central/SE Ohio Counties , up to 50% of the time.
• Central/SE Ohio Counties Counties Preferred:Ashland, Richland, Wayne, Fayette, Highland, Ross, Pike, Jackson, Licking, Muskingum, Coshocton, Fairfield, Franklin, Morrow, Washington
• Must reside in nearby counties within Central/SE Ohio Counties
• This is a hybrid position with remote work options.

Case Management Coordinator Job details

• Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment.
• Utilize critical thinking and judgment to inform the case management process.
• Conduct comprehensive evaluations of referred members' needs and eligibility.
• Coordinate and implement care plan activities and monitor progress.
• Consult with case managers and medical directors to overcome barriers to care.
• Engage members to promote participation in their healthcare decision-making.
• Document care management processes in compliance with regulatory guidelines.

Case Management Coordinator Requirements:

• Bachelor's degree in behavioral health or human services required (psychology, social work, nursing, etc.) or non-licensed master's level clinician.
• High school diploma or GED required.
• 2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focus
• Managed Care experience preferred
• Experience with Medicaid/Medicare and healthcare insurance companies
• Excellent analytical and problem-solving skills
• Effective communication, organizational, and interpersonal skills
