About the Opportunity

Spirit Homecare & Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing Home Health team .
This position is ideal for nurses who value personalized patient care , meaningful relationships , and the ability to make a direct impact in the comfort of patients’ homes.

As a vital part of our interdisciplinary team, you will collaborate closely with Registered Nurses, Therapists, and Physicians to ensure exceptional, patient-centered care.

Compensation & Schedule

• Per-Visit Rate: $28 per completed visit (average visit time: 35+ minutes)
• Flexible Options: Choose between a full caseload or PRN visits based on your schedule
• Additional Opportunities: Optional on-call shifts for increased earnings

Primary Responsibilities

• Deliver direct patient care according to physician orders and care plans
• Administer medications, wound care, and prescribed treatments safely and accurately
• Document and report patient condition changes to the RN Case Manager or physician
• Educate patients and caregivers on condition management, medications, and safety practices
• Maintain accurate, timely documentation within the EMR system
• Assist RNs with specialized nursing procedures
• Ensure full compliance with agency policies, state, and federal regulations

Qualifications

• Active Ohio LPN License (or eligibility for licensure)
• Graduate of an accredited Practical Nursing program
• 1+ year of nursing experience preferred (Home Health experience a plus)
• Knowledge of Medicare/CMS guidelines preferred
• Proficiency in electronic documentation
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

• Territory: West Chester Township and surrounding areas
• Care Settings: Patient homes, assisted living, and related facilities
• Physical Requirements: Ability to walk, bend, lift, and stand for long periods
• On-Call: Optional evening, weekend, and holiday rotations
• Communication: Maintain regular updates with the Director of Nursing
