13 Week Contract

Schedule: Monday - Friday 8 AM-5 PM

Service Area: Lorain County and surrounding areas

Requirements

Must have at least 1 year of Home Health experience within the last 3 years as an RN.

HCHB and Oasis charting experience is also required.

Duties

• Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
• Administer medications via IV infusion, injections, and other routes while ensuring adherence to aseptic technique and safety protocols.
• Manage complex wound care procedures such as suturing, ostomy care, and dressing changes, ensuring optimal healing outcomes.
• Perform specialized treatments including botulinum toxin injections for therapeutic purposes and assist with ophthalmology-related procedures when necessary.
• Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and long-term care strategies across various settings such as assisted living and nursing homes.
• Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner, maintaining compliance with HIPAA regulations.
• Provide education on disease management, medication administration, nutrition, and health coaching tailored to individual needs across diverse populations including geriatrics, pediatrics, and individuals with disabilities.

Qualifications

• Valid RN license with current state certification; experience in home health nursing preferred.
• Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-10/ICD-9 coding is advantageous.
• Critical care experience including ICU or emergency medicine is highly desirable; background in hospital medicine or trauma centers is a plus.
• Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques.
• Experience working with individuals with developmental disabilities, memory care needs, or behavioral health conditions is beneficial.
• Ability to perform complex procedures such as catheterization, phlebotomy, tube feeding management, and ventilator care confidently.
• Excellent patient assessment skills combined with compassionate communication to support patients' physical and emotional well-being. Pay: $2,050.00 - $2,150.00 per week
