Job Summary:
As a Float Licensed Practical Nurse, you’ll provide direct and indirect nursing care that supports each participant’s individualized plan. You’ll work closely with the Clinic Nurse Manager, RN, and the care team to deliver safe, accurate, and compassionate care.
This position is ideal for someone who is patient, dependable, and comfortable working within established routines. You’ll be part of a supportive team that values steady work, clear communication, and a calm, collaborative environment where participant wellbeing always comes first.
Schedule:
First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:
- Provide direct and indirect nursing care to participants.
- Collect participant data and complete required documentation accurately and according to standards.
- Assist with planning, implementing, and evaluating individualized plans of care.
- Support coordination of services by communicating participant needs and changes to the care team.
- Participate in the development of the IDT Plan of Care and ensure proper follow-through.
- Provide treatments, procedures, and care as prescribed and according to practice standards.
- Educate participants and families on health needs and care instructions.
- Complete home visits when needed.
- Document all care in the electronic medical record system.
- Support quality improvement activities.
- Follow established guidelines, policies, and participant safety protocols.
- Other duties as assigned.

- CPR and First Aid Certification (or willingness to obtain after hire — we’ll provide the training!)
- Must be at least 18 years of age.
Education and Experience:
- Graduate of an accredited nursing school and have a current state LPN license.
- One year of experience working with the frail or elderly population, or willing to undergo training to supplement this requirement, which may include overnight travel.
Requirements:
- Requires valid State driver’s license with a clean driving record.
- Successful completion of background check, physical, drug screening, TB Screening
Physical Requirements:
- Must be able to move intermittently throughout the workday.
- Must function independently, maintain confidentiality, have flexibility, personal integrity, and the ability to work effectively with personnel, regional and corporate consultants, vendors, patients/participants and support agencies.
- Is subject to lifting, carrying, and supporting Participants.
- Is subject to frequent interruptions.
- Must be constantly alert for Participant's safety.
- Must be able to drive a motor vehicle.
