Requirements

• Valid Ohio STNA/CNA license
• Unexpired CPR certification
• TB test record and physical examination (completed)
• COVID-19 vaccination card or approved exemption form
• Willingness to travel to assigned healthcare facilities
• BCI background check (if completed)

Responsibilities

• Assist patients with activities of daily living (ADLs)
• Take and record residents’ vital signs
• Provide direct patient care in accordance with care plans
• Travel to assigned nursing facilities or patient locations
• Collaborate with the interdisciplinary care team
• Maintain professional appearance and wear photo ID badge at all times
• Complete accurate documentation for both patient and facility records
• Communicate patient needs and concerns to the shift supervisor promptly
