Rehab Registered Nurse Job Details

• Estimated Weekly Salary Range: $1660 - $1700
• Assignment Type: Locum tenens (travel RN contract)
• Location: Sylvania, OH 43560 (Toledo area healthcare jobs)
• Setting: Inpatient Rehabilitation Unit (Med/Surg Rehab)
• Start Date: September 2, 2025
• End Date: December 2, 2025
• Duration: 13-week nursing contract
• Shift: Night shift, 3x12-hour shifts (18:45 – 07:15)
• Hours per Week: 36
• Weekend Coverage: Every third weekend
• Floating: Required within service line and Med/Surg, including Toledo, Flower, Bay Park, & Wildwood facilities
• Patient Ratios: Nights 1:6, Days 1:5, Weekends 1:5-6
• Charting: Epic EMR experience required
• Scrub Color: Navy
• Parking: Free onsite parking

Registered Nurse Qualifications

• Active Ohio Registered Nurse (RN) license (no pending licenses)
• Minimum 1 year of acute inpatient rehab RN experience
• Current ACLS and BCLS certifications (AHA required)
• Proficiency with Epic electronic medical records
• Ability to float within service line and to affiliate hospitals (Toledo, Flower, Bay Park, Wildwood)
• Experience caring for adult and geriatric patient populations
• Two recent professional nursing references (within last 24 months)
• COVID-19 and Flu vaccinations required (medical/religious exemptions considered)
• No local travelers within a 50-mile radius of Sylvania, OH

Registered Nurse Responsibilities

• Provide high-quality nursing care to adult and geriatric patients in a rehabilitation hospital setting
• Manage patients with diagnoses such as CVA, TBI, SCI, neuro, orthopedic, amputee, cancer/cardiac, and debility
• Perform acute inpatient rehab procedures: blood glucose monitoring, feeding tube care, NG/PEG tube management, Foley catheter insertion/removal, blood administration, IV starts, wound care, and more
• Implement and monitor individualized care plans, ensuring 3 hours of medical rehab per patient per day
• Assess, monitor, and document patient progress using Epic EMR
• Educate patients and families on care plans, safety, and rehabilitation goals
• Collaborate with interdisciplinary healthcare teams including PT, RT, pharmacy, and social services
• Float to other units and affiliated facilities within the healthcare network as needed
• Maintain infection prevention, isolation precautions, and adhere to hospital safety protocols
• Participate in orientation and required annual nursing modules
