Key Responsibilities:

• Conduct psychosocial assessments and evaluations.
• Provide individual, group, and family therapy.
• Develop and manage treatment plans.
• Document clinical notes and maintain EHR.
• Collaborate with medical and behavioral health teams.
• Participate in team meetings and case conferences.

Qualifications:

• Bachelor's or Master's in Social Work.
• Active OH license (LCSW, LICSW, or LISWS).
• EHR proficiency and strong clinical judgment.
• Empathy, cultural awareness, and teamwork mindset.
