Pay: $60-$65/Hour plus .42 a mile for patient related travel

13 Week Contract

Monday-Friday 8 AM- 5 PM

Service Area: Perry and Morgan Counties

*Must have at least 1 year of Home Health experience within the last 3 years

HCHB charting experience is also required

Responsibilities

• Conduct thorough patient assessments to develop individualized care plans that align with medical goals and patient needs.
• Deliver expert physiotherapy interventions including therapeutic exercises, trigger point therapy, and mobility training in home and outpatient settings.
• Collaborate with multidisciplinary teams to coordinate discharge planning, ensuring seamless transitions from hospital or inpatient care to home or community environments.
• Educate patients and caregivers on treatment techniques, safety measures, and ongoing health maintenance.
• Maintain accurate documentation of patient progress, treatment notes, and care plans using electronic medical record (EMR) systems.
• Assist in evaluating patient progress and adjusting therapy approaches accordingly to optimize outcomes.
• Ensure compliance with healthcare regulations, safety protocols, and best practices in home & community care environments.

Experience

• Proven experience as a licensed Physical Therapist with a strong background in home health or outpatient clinics.
• Hands-on experience with inpatient hospital settings and acute care environments is highly desirable.
• Demonstrated proficiency in care planning for diverse populations including pediatrics and geriatrics.
• Knowledge of medical terminology, physiology, and anatomy relevant to physiotherapy practice.
• Familiarity with EMR systems for documentation and patient tracking.
• Experience in discharge planning, orthopedics, trigger point therapy, and outpatient rehabilitation preferred.
