Primary Responsibilities

As a field-based LPN in home health , you’ll work closely with patients and care teams to deliver clinical services with compassion and precision. Responsibilities include:

• Provide care according to the plan of care and physician instructions
• Administer medications, wound care , and other approved treatments
• Monitor patient conditions and report significant changes to the RN Case Manager or physician
• Educate patients and caregivers on condition management and self-care techniques
• Document visits accurately and on time using our EMR system
• Support specialized procedures as directed by supervising RNs
• Ensure regulatory compliance and follow safety protocols

What You’ll Need

• Active LPN license in Ohio (or eligible to obtain)
• Graduate of an accredited Practical Nursing Program
• Minimum 1 year of LPN experience (home health experience preferred, not required)
• Knowledge of Medicare guidelines and CMS standards is a plus
• Comfortable with electronic charting and managing your own schedule
• CPR certification (current)
• Valid driver’s license , reliable transportation, and auto insurance

Work Environment & Expectations

• Coverage Area: West Chester Township, OH and nearby cities
• Work Setting: Patients’ homes, assisted living facilities, and similar environments
• Physical Requirements: Ability to lift, bend, walk, and stand throughout the shift
• On-Call Availability: Optional rotations for evenings, weekends, and holidays
• Communication: Must notify the Director of Nursing promptly about schedule changes
