Direct Support Professional (DSP) and Adult Day Support Coordinator

Performs all job duties with our agency's mission, values, and vision in mind. Ensure implementation of the Individual Service Plan (ISP) and document outcomes accurately in a timely fashion. Attend required training and update agency on any changes. Assist / perform aspects of personal care such as bathing, feeding, toileting and other self-help skills. Responsible for safeguarding consumers funds and completing appropriate documentation while on outings. Maintain confidentiality of records/ information according to HIPPA, State, Federal laws and guidelines. Maintain an environment that is healthy, safe, peaceful, and comfortable for individual(s) serviced.

We are currently seeking professional and compassionate individuals for the position of “Program Assistant” in our Adult Development and Activity Center. Our agency is also seeking professionals for Direct Support Staff in the Toledo and Bowling Green area.

Shift: 9:00 am to 3:00 pm, Monday through Friday PRN/CONTINGENT (ADS)

Direst Support Professional (DSP): 1st, 2nd, 3rd shift availability

Responsibilities:

Under the direction of the Program Coordinator, Program Assistants in the Adult Development and Activity Center Day Program help to design, lead and implement facility-based and community-based activities that are tailored to the needs, strengths, interests and preferences of consumers in order to develop consumers’ functional skills, enrich their lives through community integration and self-advocacy, further their independence through the development of employment and self-care skills, as well as provide structured opportunities for creative expression and enjoyment.Must have skill in communicating verbally and non-verbally with participants.

Assigned activities may include job coaching paid work sites, directing volunteer work at non-profit organizations; supporting development of independent living skills through activities such as using the computer and Internet; public transit, cooking and nutrition, growing vegetables; fulfilling needs for creative expression through activities such as music, dance, art, crafts, and photography; leading social recreational activities such as physical fitness, community outings, swimming and bowling; and orienting consumers in how to access community resources such as the library, parks and recreation center.
