Public Health Nurse (Registered Nurse) Complex Medical Help (CMH) Program

Full-Time (40 hours per week)

The Complex Medical Help (CMH) Program, supports children and youth with special health care needs by providing care coordination and linking families to necessary medical and social services

Key Responsibilities

• Manage a caseload of program-enrolled children.
• Conduct home visits, telehealth, or phone encounters to assess client/family needs and environments.
• Develop and implement individualized care plans and health outcomes goals.
• Educate families about their childrens conditions and coordinate services with healthcare providers and community partners.
• Participate in walk-in clinics, including health screenings, health education, and referral services.
• Administer childhood and adult immunizations in accordance with CDC and ODH guidelines, and support vaccination outreach clinics.
• Assist with inventory management and documentation related to immunization services.
• Complete timely and accurate documentation in EMR/database systems, comply with HIPAA and billing requirements.
• Engage in continuous quality improvement and stay current with best-practice standards.
• Participate in community events, training sessions, and possibly emergency response duties

Qualifications

• Bachelor of Science in Nursing (BSN) preferred; Associate degree in Nursing accepted.
• Current, active Registered Nurse (RN) license in Ohio.
• CPR certification.
• Valid Ohio drivers license, reliable transportation, and insurance coverage.
• Preferred: Minimum 23 years nursing experience, public health or case management background

Starting Salary: $23.00 an hour. Final offer is dependent on qualifications, years of nursing experience, and prior service credit in public health or related fields. The health department offers an excellent fringe benefit package and is an equal opportunity employer.
