ON CALL Schedule: Monday-Thursday 3:30 PM-2 AM.

Service Area: Ross, Pickaway, Highland, Fayette, Jackson, Highland and Vinton

Requirements

Must have at least 1 year of Hospice experience within the last 3 years as an RN.

Solid HCHB charting experience is also required.

Duties

• Assess patient needs through comprehensive evaluations, including vital signs, symptom management, and physiological assessments.
• Develop and implement individualized care plans aligned with hospice protocols and patient wishes.
• Provide direct patient care in various settings such as inpatient units, outpatient clinics, or home visits, ensuring comfort and dignity.
• Coordinate discharge planning and facilitate communication with interdisciplinary teams to ensure continuity of care.
• Monitor and document patient progress accurately in medical records while adhering to HIPAA regulations.
• Respond promptly to on-call emergencies, providing expert guidance for symptom control and crisis intervention.
• Conduct diagnostic evaluations and manage complex cases involving infectious diseases, behavioral health issues, or trauma-related conditions.

Skills

• Proven experience in critical care environments such as ICU, emergency medicine, or Level I/II trauma centers.
• Strong background in hospital medicine, medical-surgical units, or post-acute care settings like skilled nursing facilities or outpatient clinics.
• Knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for comprehensive patient assessment.
• Familiarity with managed care processes, utilization management, case management, and discharge planning procedures.
• Excellent patient service skills with the ability to handle sensitive situations compassionately while maintaining professionalism.
• Ability to perform under pressure in high-stakes environments such as emergency rooms or trauma centers.
• Competence in managing diverse patient populations including geriatrics, pediatrics, and behavioral health cases.
