Visit Nurse - Home Health (RN Preferred)
Home Care Network (HCN) is a nurse-led, locally-operated home health care agency with an excellent reputation as an employer and provider of care throughout Ohio and Indiana since 1993. We are seeking an experienced, reliable Home Health Visit Nurse (RN Preferred / LPNs considered) to provide targeted care and nursing services to our existing and new clients across Tuscarawas County, OH and surrounding areas.

- This is a long-term employment opportunity for both part-time or full-time RNs (LPNs will be considered if necessary).
- Flexible schedules and shift times are available - we build reliable, consistent schedules that fit your lifestyle and pay weekly, including weekday, evenings, and/or weekend cases.
- Those seeking full-time hours will have the option to start with part-time hours and first priority to add additional clients/shifts.
- Standard travel for client coverage up to 60 minutes from employee's home is required. Candidates only seeking cases closer to home (<30 minutes) are still encouraged to apply but please specify travel limitations in application questions.
- Mileage reimbursement is provided for travel between multiple clients.
- Home care experience, or equivalent care environments, are highly desirable.

Compensation:
- RNs: $34 - $36 per hour with bonus opportunities available.
- LPNs: $27 - $30 per hour with bonus opportunities available.
Job Summary
As a Home Health Visit Nurse , you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
- Following a physician-established plan of treatment and working under the supervision of the Director of Nursing, our Home Health Visit Nurses instruct patients and their families in proper treatment and help individuals and groups take steps to improve or maintain health.
- With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
- Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
- You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
Our nurses function as a Caregiver, Teacher, and Advocate; working in partnership with the family, our nursing staff brings a commitment to support and assist with the family in their caregiver role. As a caregiver, the nurse will provide ongoing assessment of our patient's condition response to treatments and developmental progress. As a teacher, the nurse will instruct the family on procedures, medications, safe use of equipment, and compliance with the treatments.
Reports to: Regional Administrator, or Clinical Administrator/Director of Nursing
Qualifications
- Graduate of an accredited School of Nursing.
- RNs highly preferred.
- Minimum of 1 year experience as a paid RN or LPN with a home health agency or equivalent in-home care environment is preferred but not required.
- An active RN or LPN License with ability to practice in the State of Ohio.
- Active CPR and First Aid Certification, or willingness to renew prior to start.
- Case Management and care team / staff management experience is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
- Current Driver's License and reliable transportation.
