Registered Dietitian

Position Overview:
Monitor My Health is seeking a qualified and motivated Registered Dietitian (RD) to support our Diabetes Prevention Program (DPP). The RD will provide one-on-one nutritional counseling to program participants via phone, helping individuals make sustainable lifestyle changes to reduce their risk of type 2 diabetes.

Key Responsibilities:

• Conduct individualized nutrition counseling sessions with participants following completion of their 4th DPP class session
• Review participant nutritional assessment forms prior to sessions
• Provide evidence-based guidance tailored to each participant’s needs and goals
• Complete the following session schedule for each participant:
• Initial session (30 minutes)
• Follow-up session (15 minutes) – 4 weeks after the initial session
• Final session (15 minutes) – 6 weeks after the second session
• Maintain timely and accurate documentation of all interactions
• Communicate effectively with program staff as needed

Program Details:
The RD will support participants across multiple classes:

• Class 1: 13 participants (4th session scheduled for April 22, 2026)
• Class 2: 16 participants (4th session completed on April 1, 2026)
• Class 3: 8 participants (4th session scheduled for April 24, 2026)
• Class 4: 16 participants (4th session completed on April 6, 2026)

Qualifications:

• Registered Dietitian (RD) certification/license (required)
• Experience in nutrition counseling, diabetes prevention, or chronic disease management preferred
• Strong communication and motivational interviewing skills
• Ability to work independently and manage scheduling with participants
• Comfortable conducting sessions via phone

Schedule & Commitment:

• Flexible, based on participant availability
• Candidates may indicate the number of classes they are available to support
