  • Homecare Team Manager in Coldwater, OH
  • Care Center Team Manager in IPU at Wayne Hospital Greenville, OH

Summary of Position: The Team Manager is directly responsible for providing direct supervision and clinical oversight of hospice clinical staff and serves as their clinical resource.

Education and Experience:

  1. Graduation from an approved School of Nursing.
  2. Current RN licensure in the State of Ohio.
  3. Minimum of three years of experience in Medical/Surgical, Oncology or community health nursing.
  4. Hospice experience preferred.
  5. Minimum of one year of management experience.

Special Skills and Requirements:

  1. Shall have a valid driver’s license.
  2. Excellent customer service skills.
  3. Detail oriented with excellent communication, documentation, and organizational skills.
  4. Ability to follow instructions.
  5. Ability to work independently without supervision.
  6. Travel to other sites as needed. Travel may be in inclement weather.
  7. Ability to lift 25 lbs of supplies or equipment.
  8. Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.

Responsibilities of Position:

  1. Assist with the development and implementation of Agency policies and procedures.
  2. Manage departmental budget and control expenditures.
  3. Enforce departmental vision, goals, and implementation plan.
  4. Build and maintain an effective team.
  5. Participate in Agency survey. (ie. Ohio, Joint Commission, Finance, etc)
  6. Provide expert leadership regarding optimum symptom management and overall care provided.
  1. Collaborate with Medical Director, primary physicians and/or consulting physicians regarding appropriateness for re-admission or re-certification and regarding appropriate symptom management.
  1. Participate in Interdisciplinary Team Conferences.
  1. Participate in the development, review, and evaluation of the patient’s Plan of Care across all levels and settings of routine hospice care, including crisis care, end of life care, and GIP through ongoing review of clinical records and chart reviews.
  2. Monitor quality of care and appropriateness of care provided in all settings whether that is in the patient’s home, skilled nursing, or assisted living setting.
  3. Monitor clinical documentation for timeliness, appropriateness and compliance with Agency policy, accreditation bodies, and governing requirements.
  1. Assist Director of Nursing with additional documentation requests from Payer Service.
  1. Evaluate staff productivity. Make supervisory visits to evaluate staff competency annually.
  2. Participate in visits as needed.
  1. Work evenings and weekends as administrative on call.
  2. Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  1. Other duties as assigned.
