Responsibilities for a Wellness Coordinator with Phoebe Allentown:

Assess the health and psychosocial needs of prospective and current program members. Develop an individual wellness plan for the coordination of care services, proactive wellness interventions, and oversee the implementation of the developed wellness plan for program members. Establish and maintain linkages with agencies and service providers as well as monitor the services being administered to the program members. Participate as an active member of the interdisciplinary team to identify program member needs and provide input. Provide education to program members as needed. Develop and implement wellness programming for program members that are both individualized and group. Participate in educational seminars with other Pathstones’ staff.

Requirements for a Wellness Coordinator with Phoebe Allentown:

1. The ability to deliver excellent customer service to all residents, families, and coworkers.
2. Bachelor’s degree in Social Work; Psychology; Human Services; Counseling, Communication or related field.
3. Must have valid driver license. Ability to work approximately 20-25 hours per week on flexible schedule to meet needs of program members.
4. Minimum one (1) year of care management; counseling; social work and/or related experience in assisting aging adults with care coordination and wellness programming.
